Non-Management Evaluation Form for Hourly & Professional Employees
Hourly: Non-white collar employees who are paid on an hourly basis and receive overtime for all hours worked above 40 in a work week. Hourly paid employees do not qualify for the Executive, Administrative or Professional exemption under the Fair Labor Standards Act.
Professional: White collar employees who are paid on a salary basis and must perform work requiring advanced knowledge acquired by a prolonged course of specialized intellectual instruction. Professional employees qualify for the professional exemption under the Fair Labor Standards Act.
Executive: White collar employees who are paid on a salary basis and must regularly direct the work of at least two or more employees and have hire/fire authority. Executive employees are exempt from overtime regulations.
Administrative: White collar employees who are paid on a salary basis and must have a primarily duty of office or non-manual work related to management or general business operations. Administrative employees are exempt from overtime regulations.
The purpose of the exit interview is twofold. First, it is to help determine the reason for the employee’s departure and to determine if it would have been possible to keep the employee when appropriate. Second, it is to evaluate the employee’s thinking in relationship to us as employer. This will assist us in determining whether we as an employer need to make changes.
