Welcome to the CSGPO Interactive Forms help page
From our form pages you can request videos from our catalog, sign up for workshops, request purchases orders, file reports and surveys and provide other information that may be required from time to time.

On this page you will find information on the legend of symbols marking the forms, System requirements and some Step-By-Step instructions on using the forms

You will need Adobe Acrobat Reader, Version 4.0 or higher to view, print and submit files on this web site.  If you do not have the Acrobat Reader you can download one free from the Adobe Site.

Legend of Symbols

    Forms with a Green pin are INTERACTIVE - they require Acrobat reader 4.0 or higher and are submitted on-line.

If you require a copy of this form After filling in the form print it using the print command from the Acrobat Tool Bar (picture of printer) BEFORE you click on the submit button.

 
    Forms with a Green pin are INTERACTIVE - they require Acrobat reader 4.0 or higher but can not be submitted on-line. It most likely require a signature.

Forms marked with this pin can be filled in within your browser window but can NOT be submitted on-line.  After filling in the form print it using the print command from the Acrobat Tool Bar (picture of printer)  then sign and mail this form -make a copy for your records.

 
    Forms with a red pin are not interactive They are for Printing, Filling Out, and then Mail or Fax. They may also be publications.  Print it using the print command from the Acrobat Tool Bar (picture of printer)

System Requirements:

This site allows you to fill out and submit fully formatted forms while online.  To use this service you will need the following:

  • Acrobat Reader version 4 or above for interactive forms. - (version 3 will not work)
  • Web browser with Acrobat Reader installed to open inside your browser window.
  • Acrobat Reader version will work for publications ONLY.

Step-By-Step instructions

If your system meets the above-named requirements, you are ready to use the interactive forms. Follow the steps below.

 

     

1.

Select the form you need.
     

2.

Provide the information requested in the yellow highlighted areas.
     

3.

Print the form using the print command on the acrobat  tool bar.
     

4.

Submit the form using the submit button at the bottom of the form
     

5.

You will receive a conformation e-mail after you form has been received and processed.
     

6.

You will need to Sign and Mail back the printed form - keep a copy for your records and submit the original per instructions.
     

7.

Follow the instructions with the form for distribution and submission.
     

8.

USE The BACK Button of your browser to exit the reader and return to the page you were using when you requested the Form or Document.
