In
addition to to registering for Cast Your Nets!,
every young person in attendance must have a
signed medical release form.
We provide an example form on this website, but
there are fields on the second page that must be
completed by the group leader before forwarding
to the youth to fill out.
The required fields are: Church Agency and Group Leader
(with emergency contact).
If you choose to organize transportation, you
must also fill out Type of Transportation and
Meeting Place lines.
You may download the medical release form as an
editable .pdf on which you can fill in your
church information then print out
on your own computer for distribution, or as an
MS Word .doc that you can edit and save
electronically to distribute to your
participants. The two types of forms are below:
Registration is not available for
Cast Your Nets! Dayton (October 2010) or
Cast Your Nets! Cincinnati (January 2011)
at this time.
