v Your emblem and certificate orders must be reviewed and processed by the diocese in which they are earned. We are happy to continue to serve you in this way.

v We need to receive an Intent to Begin page from each group as it begins working on a religious emblems program. This will insure that an adequate quantity of emblems are on hand before the awards ceremony. The Intent to Begin page may be found in the adult leader's manual for the Ad Altare Dei and Pope Pius XII emblem programs. The adult leader should copy that page before filling it out, keeping the original in the book for use with future groups.

Please mail your orders for emblems and certificates to the Archdiocese of Cincinnati Office of Youth and Young Adult Ministry (OYYAM). Include the completed order form, a check payable to the “Archdiocese of Cincinnati”, the completed and signed application pages (detached from each activity book), along with any related evaluation forms, to your local OYYAM (see order form for address information). As we are not equipped to take credit card payments, please send a check or money order (no cash through the mail please).
