Registration Details
There is no online registration for Junior High Leadership C.A.M.P. No one will be considered registered until
three
necessary forms are received. They are:
1.)
The Registration Form
(On flyer)
This is found on the flyer. To download it,
click on the link on the left that says, Flyer,
or on the link above.
2.)
Archdiocese of Cincinnati
Permission, Release and Power of Attorney Form
This is the standard medical release form for all archdiocesan
activities. Be sure parents print out both sides of this form
and fill out the medical information completely.
3.)
Guidelines for Behavior
Agreement
This is a code of conduct for each participant. Be sure your
youth and parents actually read this form before signing.
Cost is $105 per person if
registration if received by May 7. After that, cost is $125
(includes 4 meals, lodging, retreat materials and t-shirt).
