PROCEDURES FOR ORDERING

Program Books, Medals and Patches

Program Book Orders
Religious recognition program books must be ordered through the National Federation for Catholic Youth Ministry (NFCYM). There are three ways to order:

1.  Go to the NFCYM Online Store and place your order using a credit card.

2.  Call the NFCYM: 202-636-3825 (Customer Service), 202-526-7544 (Fax)

3.  Go to the NFCYM Online Store, then print out your order and mail it with a check to:      National Federation for Catholic Youth Ministry, Inc., Order Department - Suite 40,      415 Michigan Avenue, NE, Washington, DC 20017-4503

4.  Send an Email to info@nfcym.org.

All orders under $100 dollars must be prepaid by check or credit card. Orders over $100 may be billed. Please note that credit card information should never be placed in an email as it is an unsecured means of communication.

Medal and Patch Orders (PLEASE READ CAREFULLY)
All medals and patches must now be purchased directly from the NFCYM. HOWEVER, all applications must be reviewed and processed by the Archdiocese of Cincinnati Office of Youth & Young Adult Ministry (OYYAM) PRIOR to ordering.

1.  Send the completed and signed original application page (detached from each workbook) in addition to the completed Request for Authorization Letter to your nearest regional Office of Youth & Young Adult Ministry (addresses listed below). Please note that only original signed applications removed from workbooks are valid. Copies are not acceptable.

2.  If the applications are in order, the Office of Youth & Young Adult Ministry will provide a Letter of Authorization to the leader to buy the appropriate number of medals and patches directly from the NFCYM. Each Letter of Authorization expires in 30 days from the date on the Letter. If requested, the OYYAM will email an electronic authorization letter to the NFCYM on your behalf. 3.  Leaders may then order medals and patches directly from the NFCYM by mentioning that their Letter of Authorization was sent via email from the Archdiocese of Cincinnati Office of Youth & Young Ministry.  If a leader requests a hard copy of their Letter of Authorization, then it must be included in the mailing to the NFCYM along with payment for the entire order. Contact information for the NFCYM is listed above. The prices of medals vary, but all patches are $3.00 each.  Overall, there is a $1.50 discount for ordering the same program's medal and patch together.  Prices of medals are: Family of God $4.50; I Live My Faith $4.50; Marian $8.50; Spirit Alive $10.00. These prices are subject to change by the NFCYM without notice.
